Thank you for participating in the UDFC Back-to-School Flash Sale! Please review the following terms carefully before purchasing. By signing below, you acknowledge and agree to all promotion requirements.
PROMOTION DETAILS
This promotional offer provides four (4) Myofunctional Therapy sessions at the promotional rate available at the time of purchase. Functional Reassessments are excluded from this deal.
The promotional price is time-sensitive and increases every 24 hours during the promotional period. Once a pricing period expires, the previous promotional rate will no longer be available.
ELIGIBILITY REQUIREMENTS
To qualify for this promotion:
The promotion is available to children and adults.
A Comprehensive Myofunctional examination must have been completed within the previous 12 months and you are a patient of record.
The promotion is valid at the Chino Hills location only. Mondays, Fridays and two Saturdays a month.
All four (4) sessions must be scheduled at the Chino Hills location.
Availability is limited and appointments are subject to UDFC scheduling availability. Please check with patient care coordinator current time slots that are open prior to purchase.
A maximum of three (3) promotional packages may be purchased per patient.
SCHEDULING REQUIREMENTS
All four (4) sessions must be scheduled at the time of purchase to receive the promotional rate.
Sessions are subject to UDFC's normal scheduling availability.
Appointments must be attended according to UDFC's standard cancellation and rescheduling policies.
Failure to schedule all four sessions at the time of purchase may result in loss of promotional eligibility for the unscheduled sessions.
PAYMENT & PROMOTIONAL RATE
Payment in full is required at the time of purchase.
The promotional rate is based on the price available at the exact time the package is purchased.
Once the 24-hour promotional period ends and the price increases, the previous rate cannot be reinstated.
Promotional pricing cannot be combined with other discounts, promotions, or offers.
TRANSFERS, REFUNDS & EXTENSIONS
Promotional packages are non-transferable and may only be used by the patient for whom the package was purchased.
Promotional packages are not eligible for cash redemption.
Promotional pricing cannot be retroactively applied to previous purchases.
Promotional packages cannot be transferred to another patient or family member.
Promotional packages are not eligible for refunds based solely on a change of mind or failure to schedule appointments.
Promotional appointments may not be extended beyond the package expiration date, if an expiration date is specified by UDFC.
CLINICAL CONSIDERATIONS
Purchase of this promotional package does not guarantee a specific clinical outcome.
Myofunctional Therapy recommendations and treatment plans are based on the patient's individual needs and clinical findings. UDFC reserves the right to determine whether a patient is clinically appropriate for continued Myofunctional Therapy.
If a patient's clinical needs change or additional services are recommended, those services may be subject to separate fees.
CANCELLATION & MISSED APPOINTMENTS
All appointments remain subject to UDFC's standard cancellation, rescheduling, and missed-appointment policies.
A missed appointment or late cancellation may result in the applicable session being forfeited according to UDFC's established policies. Credit card must be on file. Cancellation requires more than 48 hour notice. A no show or late cancel fee is half the cost of the standard session price which is $75. You signing this agreement means you are aware of the cancellation policy and your card on file with incur an additional fee of $75 should you not cancel within the 48 hour window.
PROMOTION AVAILABILITY
This promotion is available for a limited time and while appointments remain available.
UDFC reserves the right to end the promotion once the advertised promotional period has expired or available promotional appointment capacity has been reached.
The promotional rate is not guaranteed after the applicable 24-hour pricing period ends.
PATIENT ACKNOWLEDGMENT & AGREEMENT
By signing below, I confirm that:
☐ I have reviewed and understand the eligibility requirements for the Back-to-School Flash Sale.
☐ I understand that my Myofunctional examination must have been completed within the previous 12 months.
☐ I understand that all four (4) sessions must be scheduled at the Chino Hills location.
☐ I understand that all four (4) sessions must be scheduled at the time of purchase.
☐ I understand that the promotional price increases every 24 hours and that the rate available at the time of purchase is the rate that will apply to my package.
☐ I understand that promotional packages are non-transferable and subject to UDFC's standard scheduling and cancellation policies.
☐ I understand that a maximum of three (3) promotional packages may be purchased per patient.
☐ I have had the opportunity to ask questions regarding the terms of this promotion and agree to abide by the requirements above.
